- 04K: Evidence of rats or live rats.
- 04L: Evidence of mice or live mice.
- 04M: Live roaches.
- 04N: Filth flies or food/refuse/sewage-associated flies.
- 08A: Facility not vermin proof, with harborage or openings that let pests in.
When pests lead to a closure
A single sighting usually results in points. Widespread evidence in food preparation or storage areas can be treated as a public health hazard and lead to the restaurant being closed until it is cleaned and re-inspected.
